Opokan 7.5mg

Opokan belongs to a group of medicines called non-steroidal anti-inflammatory drugs (NSAIDs). Opokan has anti-inflammatory, analgesic and antipyretic properties. Opokan is used in osteoarticular and muscular pain (e.g. backache, back pain, knee pain) in the course of rheumatoid and degenerative joint diseases.

Symptoms

Short-term treatment of exacerbations of rheumatoid symptoms, such as:

  • rheumatoid arthritis,
  • juvenile rheumatoid arthritis,
  • ankylosing spondylitis.

Active substance: Meloxicamum

Composition

Active substance: one tablet contains 7.5 mg meloxicam.

Other ingredients: microcrystalline cellulose, lactose monohydrate, pregelatinized starch, maize starch, sodium citrate, colloidal anhydrous silica, magnesium stearate.

Dosage

Always take this medicine exactly as described in this leaflet or as directed by your doctor or pharmacist. Check with your doctor or pharmacist if you are not sure.
Recommended dose:
1 tablet (7.5 mg) daily.
Elderly patients: 1 tablet (7.5 mg) daily.
Patients with renal insufficiency: a daily dose of 7.5 mg should not be exceeded.
Patients with liver disease: a daily dose of 7.5 mg should not be exceeded.
Use in children: Opokan should not be used in children under 15 years of age.
Method of administration:

  • Opokan is taken orally with a meal, washed down with water.
  • Opokan should be taken once a day (in a single dose).
  • The maximum daily dose is 7.5 mg (1 tablet).

If symptoms worsen or do not improve after 7 days, please contact your doctor.
If you take more Opokan than you should
If you take more than the prescribed dose, tell your doctor or pharmacist immediately.
Symptoms of overdose may occur:

  • nausea, vomiting, abdominal pain
  • headache, dizziness, insomnia or sleepiness
  • hypertension,
  • kidney problems
  • liver dysfunction
  • decrease in breathing rate or shortness of breath, respiratory arrest,
  • coma,
  • seizures
  • cardiac arrest
  • allergic reaction (see section 4: Possible side effects).

If you forget to take Opokan:
Continue using the medicine without increasing the next dose.
Do not use a double dose to make up for a forgotten dose.
If you stop taking Opokan:
If you have any further questions on the use of this medicine, ask your doctor or pharmacist.

Use of the drug in children and adolescents
Do not use in children and adolescents under 15 years of age.

When should you not use Opokan?
  • If you are allergic to meloxicam or any of the other ingredients of this medicine.
  • If you are hypersensitive to any other non-steroidal anti-inflammatory drug (NSAID), e.g. acetylsalicylic acid.
  • If you have experienced attacks of breathlessness or asthma, hives, nasal polyps or angioedema while taking other non-steroidal anti-inflammatory drugs (e.g. acetylsalicylic acid).
  • If you have severe liver problems.
  • If you have severe kidney failure and you are not receiving dialysis treatment
  • If you have an active or recurrent peptic ulcer in the stomach and / or duodenum (who have had at least two episodes of proven ulceration or bleeding).
  • If you have had a cerebral haemorrhage or other bleeding, e.g. from the gastrointestinal tract.
  • If you suffer from severe heart failure.
  • In women in the third trimester of pregnancy and breastfeeding.
  • In children under 15 years of age.

Warnings

Talk to your doctor or pharmacist before taking Opokan:

  • if you have had a sudden allergic reaction (known as anaphylactic shock) within 20 minutes of taking the medicine,
  • if you have a stomach ulcer and / or duodenal ulcer or inflammation of the lining of the esophagus or stomach. This medicine can be given to patients whose disease has completely healed,
  • if you have bronchial asthma, as the drug may cause bronchospasm,
  • if you have diabetes, high blood cholesterol and high blood pressure; the risk of a heart attack or stroke increases,
  • if you suffer from mild to moderate heart failure (manifested by edema),
  • if you have ischemic heart disease, peripheral arterial disease or cerebrovascular disease, as this increases the risk of an embolism
  • if you have bowel disease (ulcerative colitis or Crohn’s disease) as the risk of gastrointestinal bleeding is increased. If gastrointestinal bleeding occurs, treatment should be discontinued immediately.
  • if you suffer from impaired kidney function or a reduced volume of circulating blood; kidney function should be checked regularly,
  • if you have liver problems; the drug may cause a slight, transient increase in liver enzymes in the blood (AST and ALT),
  • if you are over 65, as the risk of kidney, liver and heart problems is increased. The doctor will order regular examinations,
  • if you are planning to become pregnant or have difficulty becoming pregnant because this medicine is affecting your fertility. In this case, the drug should be discontinued.
  • Opokan may mask the symptoms of an existing infection.

Taking medicines like Opokan may be associated with a slightly increased risk of heart attack (“heart attack”) or stroke. This risk is increased by long-term ingestion of high doses of the drug.

Higher doses and longer duration of treatment than recommended should not be used.

If you have heart problems, have had a stroke, or suspect that you are at risk of these disorders (e.g. high blood pressure, diabetes, high cholesterol, smoking), discuss treatment with your doctor or pharmacist.
Children and youth:

  • Do not use in children and adolescents under 15 years of age.

Opokan contains lactose. If you have been diagnosed with an intolerance to some sugars, contact your doctor before taking the drug.

Opokan in pregnancy

If you are pregnant or breastfeeding, think you may be pregnant or are planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.

  • Opokan may adversely affect your unborn baby. It is not recommended to use the drug during the first and second trimesters of pregnancy, unless the doctor considers it absolutely necessary. If Opokan is taken by women during the first and second trimesters of pregnancy, the dose should be kept as low and for the shortest possible time.
  • Opokan must not be used in the last 3 months of pregnancy (third trimester).
  • Opokan passes into breast milk, so you must not breast-feed while using this drug.
  • Opokan may adversely affect female fertility and make it difficult to get pregnant. Do not use in women who are having difficulty conceiving or undergoing infertility testing.
Overdosage

If you take more than the prescribed dose, tell your doctor or pharmacist immediately.

Symptoms of overdose may occur:

  • nausea, vomiting, abdominal pain
  • headache, dizziness, insomnia or sleepiness
  • hypertension,
  • kidney problems
  • liver dysfunction
  • decrease in breathing rate or shortness of breath, respiratory arrest,
  • coma,
  • seizures
  • cardiac arrest
  • allergic reaction
Other medicines and Opokan

Tell your doctor or pharmacist about all the drugs you are taking or have recently taken, and about any drugs you plan to take.
Opokan should not be used together with the following medicines:

  • other non-steroidal anti-inflammatory drugs, including salicylates (e.g. acetylsalicylic acid in a single dose of 1 g and a daily dose of 3 g), concomitant use with Opokan may increase the risk of gastrointestinal ulceration and bleeding,
  • diuretics (which increase urine production, e.g. furosemide), concomitant use with Opokan may cause severe renal failure in dehydrated patients.Patients receiving Opokan and diuretics concomitantly should be given plenty of fluid (to drink and / or via intravenous drips). Before starting treatment, the doctor will check the function of the kidneys,
  • oral anticoagulants (used to prevent blood clots); simultaneous use with Opokan may increase bleeding,
  • drugs to lower blood pressure:
    • angiotensin converting enzyme inhibitors (e.g. captopril, enalapril),
    • angiotensin II receptor antagonists (so-called sartans),
    • beta blockers.

Concomitant use of the above-mentioned drugs with Opokan may cause severe impairment of kidney function and reduce their therapeutic effect.

  • ciclosporin (a medicine used in organ transplant patients); your doctor will check your kidneys regularly,
  • contraceptives intrauterine; Opokan may reduce their effectiveness.You need to use an additional method of contraception, such as using a condom,
  • glucocorticosteroids (hormones of the adrenal cortex); simultaneous use with Opokan may increase the risk of gastrointestinal ulceration and bleeding,
  • lithium (a medicine used to treat certain mental illnesses),
  • methotrexate (a cytostatic drug used to treat cancer)
  • oral medications to treat diabetes
  • cholestyramine (a medicine to lower cholesterol in the blood).
Side effects

Like all medicines, this medicine can cause side effects, although not everybody gets them.

The following side effects may occur.
Common (affects 1 to 10 in 100 people):

  • nausea, vomiting, abdominal pain, gas,
  • constipation, diarrhea
  • anemia,
  • a feeling of emptiness in the head,
  • headaches,
  • rash (itchy red lumps on the skin)
  • swelling, including in the ankles.

Uncommon (affects 1 to 10 in 1,000 people):

  • gastrointestinal bleeding (vomiting containing blood or coffee grounds – similar to coffee grounds, black stools or blood in stools)
  • gastric ulcer and / or duodenal ulcer,
  • esophagitis,
  • inflammation of the oral mucosa,
  • transient liver dysfunction
  • reduction in the number of white blood cells
  • reduction in blood platelets (may increase risk of bleeding and bruising)
  • decrease in the number of granulocytes (a type of white blood cell that may decrease your immunity)
  • dizziness, tinnitus,
  • -somnolence,
  • feeling your heart beating fast
  • increase in blood pressure
  • reddening of the face with a feeling of heat,
  • hives (pale pink, itchy blisters on the skin)
  • kidney problems (e.g. increased levels of creatinine or urea in the blood).

Rare (affects 1 to 10 in 10,000 people):

  • gastritis,
  • ulcerative colitis,
  • hepatitis,
  • allergic reaction for up to 20 minutes after taking the medicine (rash, severe swelling of the face, mouth or throat, difficulty breathing, speaking or swallowing, a sharp drop in blood pressure, slow or fast heart rate, paleness, restlessness, sweating, dizziness, loss of consciousness) stopping breathing and heart beat)
  • mood changes
  • insomnia, nightmares,
  • disorientation (confusion),
  • visual disturbances (e.g. blurred vision)
  • asthma attacks (wheezing, coughing, difficulty breathing)
  • occurrence of blisters on the skin and / or mucous membranes that, when ruptured, form painful wounds, often at the same time as fever, muscle and joint pain (this is called Stevens-Johnson syndrome)
  • exfoliation of large areas of the epidermis and its necrosis,
  • erythema multiforme (glaucous red patches on the skin, sometimes with blisters)
  • sensitivity to light,
  • an allergic reaction which causes swelling of the face, throat and limbs (called angioedema)
  • kidney failure (manifested by decreased urine output, blood in the urine, cloudy urine, ankle swelling, nausea, vomiting).

Exacerbation of symptoms may occur especially in patients with hypersensitivity to acetylsalicylic acid or other non-steroidal anti-inflammatory drugs and in the elderly. Opokan may cause a slightly increased risk of a heart attack or stroke.
